This process will go quickly if you complete the demographics form and past medical history form ahead of time. We'll also give instructions on how to set up your secure, online My Health Record account. You will also be asked to confirm your medical insurance.
We'll take your vital signs, and you will meet with your doctor or nurse practitioner to discuss your medical history and any problems you may be having. He or she may perform a physical exam, and we may draw blood if necessary. Our in-house phlebotomist can draw your blood so you won't need to visit an outside lab.
Your lab test results will be posted on My Health Record. If you are visiting us for an annual exam, which is recommended for all women, you will undergo a pap smear and a breast exam. At this time, we can provide necessary prescriptions or medical orders, such as for a mammogram or bone density test.
